Job Description

About the Role

MAP is seeking a Rehabilitation Point Coordinator to lead a multidisciplinary team, oversee case assessments and referrals, and ensure effective follow-up and data management. The role includes managing assistive device distribution, maintaining accurate records, and coordinating with logistics and services to meet beneficiary needs. 
 
About You
The ideal candidate holds a degree in psychology, social work, sociology, or a medical-related field, with experience in rehabilitation and case management. The role requires strong leadership in managing multidisciplinary teams, solid knowledge of disability rights and assistive technologies, and excellent communication, organisational, and data management skills. Fluency in English and Arabic is essential.

Responsibilities
Receive cases and conduct thorough assessments using the case form/general assessment form.
Supervise and manage a multidisciplinary rehabilitation team and ensure accurate data collection and registration related to beneficiary and their sessions.
Manage the referral process by coordinating with relevant services and ensuring timely and appropriate follow-up for each case.
Collaborate closely with the rehabilitation team to monitor the progress of referred cases, ensuring that all needs are being addressed effectively.
Regularly update and communicate with the team regarding the status and needs of ongoing cases, addressing any challenges that arise.
Work with the logistics team to arrange the timely and organised delivery of these items, keeping track of inventory and distribution records.
Maintain accurate and up-to-date records of all cases, including assessment forms, referral documentation, and follow-up notes.
Prepare and submit regular reports on case management activities, highlighting key achievements, challenges, and recommendations for improvement.
Ensure that all data is handled confidentially and that reports are submitted to the line manager in a timely manner.
Manage and monitor the distribution of assistive devices and other non-food items for the beneficiaries.
Contribute to developing relevant measurement and data documentation tools and keep reflecting the collected data in the online system.
Requirements
We seek a candidate with strong literacy, numeracy, and analytical skills, backed by critical thinking, problem-solving, and effective communication, demonstrated through relevant qualifications or experience. Holding a degree in psychology, social work, sociology, or medical-related field, from an accredited educational institution.
Experience in rehabilitation settings and working with multidisciplinary rehabilitation team.
Experience in case management, particularly in a healthcare setting.
Extensive experience in team management.
Good knowledge of multidisciplinary rehabilitation services approaches in emergency.
Knowledge of adaptive equipment, assistive devices/technologies, and accessibility modifications to enable persons with disabilities to live, work, and engage in the community.
Familiarity and knowledge of the rights of persons with disabilities, relevant local, national, and international laws and regulations that protect the rights of persons with disabilities, such as anti-discrimination laws, accessibility standards, and provisions for reasonable adjustments.
